Liability Waiver & Release I, the undersigned participant, or parent/legal guardian of the participant if the participant is under, 18 years of age, acknowledge that participation in soccer training, camps, clinics, games, practices, tournaments, and related activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, or death. SoCal Ballerz FA is a non profit organized organized exclusively for charitable and educational purposes. I voluntarily assume the risks associated with participation in activities organized by SoCal Ballerz FA. I hereby release, waive, discharge, and hold harmless SoCal Ballerz FA, their directors, officers, employees, coaches, volunteers, contractors, and affiliates from any and all claims, liabilities, or causes of action arising from participation. Photo & Video Release I grant permission for SoCal Ballerz FA to use photographs, video recordings, and other media of the participant for nonprofit, promotional, educational, marketing, grant reporting, and social media purposes without compensation. I understand that these materials may be used in pint and digital formats and waive any right to inspect or approve the finished product.
